OvercrowdingThere is only so much room in your mouth for your teeth. Ideally, there is exactly the right amount of room for all your natural teeth to fit properly. If there isn’t, room must be made for your teeth to fit and align properly. If your jaw and gum structure lacks adequate space for your teeth, they may become misaligned, settling into incorrect positions. Making Way for Permanent TeethNormally, primary or “baby” teeth loosen and fall out in time to allow space for permanent teeth to come in. But if the primary tooth is showing no sign of loosening, the permanent tooth will end up erupting at an incorrect location, causing significant alignment problems. The primary tooth may need to be extracted to prevent this. If there are alignment problems that need to be corrected with traditional braces or Invisalign, an extraction might be called for to help facilitate this correction. Braces cannot provide proper correction if there is insufficient space available. Wisdom TeethVery often, the four rearmost molars, also known as wisdom teeth, must be extracted. Reasons for this include insufficient space available for them to occupy, pushing other teeth out of their correct location, or causing inflammation and pain in the gumline. Wisdom teeth can become impacted. They are unable to erupt normally, sometimes due to growing at an angle instead of straight up and down. They can cause pain and damage to the second molars. Sometimes the roots of a wisdom tooth may end up straddling a nerve in the jaw, and could cause nerve damage unless removed. Preparation And ProcedureOur skilled oral surgeons will use methods that maximize your comfort. Depending on your preference, you may be placed under a general anesthetic, causing you to sleep though the procedure. Or you may be given a local anesthetic, which numbs the area receiving the treatment while allowing you to remain awake. In the second example, your gums and surrounding area will be numbed by first applying a topical agent, then injecting a second numbing agent for the tooth itself. Most extractions are accomplished by gently and firmly moving the tooth back and forth to disconnect it from the supporting ligament and bone. In some cases, the tooth may need to be separated into several pieces to allow extraction.
